ELIGIBILITY
New employees who start work with NMC become eligible for benefits on the first day of the month following date of hire.
Full-Time and Regular Part-Time employees are eligible to enroll.
Employees are allowed to elect 1x, 2x or 3x their annual salary.
Employment Classification:
Full Time (FT)= 36 to 40 hours per pay week
Regular Part Time (RPT)= 30 to 35.5 hours per pay week
Part Time (PT)= 20 to 29.5 hours per pay week
Regular Per Diem (RPD)= at least 8 hours per pay period
Per Diem (PD)= as needed

NMC offers employees Short Term Disability coverage through Reliance Standard. This plan is designed to provide a continuation of benefits to you in the event of either sickness or accident.
ELIGIBILITY

Current employees who transfer from a non-benefit-eligible position to a benefit-eligible position follow the New Employee guidelines above.

My Supplemental Life Benefits

Employees are offered Voluntary Life Insurance and Accidental Death & Dismemberment Program in which employees can obtain additional term life insurance coverage for themselves and their dependents.
Supplemental life insurance coverage can be elected in conjunction with the Basic Life coverage. If an employee elects life insurance after they are initially eligible, Evidence of Insurability will be required and the coverage will have to be approved. Full Time and Regular Part Time employees are eligible to enroll.

Supplemental Life Insurance (100% Employee Funded; age rated premiums)
- Choice of coverage from 1x to 3x annual base salary to a maximum of $500,000 combination of Basic Life and Supplemental Life
- Benefits reduce by 35% upon reaching age 70; 50% upon reaching age 75
- Employee required to give satisfactory evidence of insurability for supplemental life amounts over $250,000
Dependent Life Insurance (100% Employee Funded)
- Employees may elect coverage on their spouse for $10,000, $20,000, $30,000 or $40,000
- Employees may elect coverage on their children for $5,000 or $10,000

My FSA Benefits

Flexible Spending Accounts (FSA) allow employees to set aside money on a pretax basis.
- Funds in a medical FSA may be used to reimburse you for expenses related to medical, prescription, dental, vision and limited over the counter expenses.
- Employees may contribute up to $2,750 into a medical FSA in 2021.
- Funds may be used to pay for eligible expenses incurred by any tax dependent.
- Funds are available in their entirety on the effective date of the FSA.
*Employees who choose the HSA Medical Plan and elect to contribute to a Health Savings Account (HSA) may elect a medical FSA, but may only use funds to reimburse dental and vision expenses. This is considered a Limited Purpose FSA.
- A Dependent Care Account (DCA) is an FSA that allows employees to put funds away on a pre-tax basis to pay for costs associated with day care and day camp expenses for their children under age 13.
- DCA funds are available for distribution as they are deposited into the account.
- Employees may elect up to $5,000 per household ($2,500 if married and filing separately).
Employees will receive an Allegiance Debit Card if they enroll in a Flexible Spending Account, FSA or DCA.
CLAIMS FILING PROCEDURE:
Written notice of the Participant’s or the Dependent’s claims (proof of claims) must be given to the Contract Administrator as soon as is reasonably possible but prior to the end of the Run-out Period, which is ninety (90) days after the end of the Plan Year. Claims submitted after the Run-out Date will be denied.

My HSA Benefits

Full Time, Regular Part Time & Part Time employees are eligible to contribute to a Health Savings Account if enrolled in the HSA Medical Plan which is a Qualified High Deductible Health Plan. All employee’s contributions can be deducted pre-tax & may be changed throughout the year as long as the total contribution does not exceed the Federal Maximums.
My Critical Illness and Accident Benefits

NMC offers employees Worksite Benefits through Reliance Standard. These plans are designed to provide a continuation of benefits to you in the event of either sickness or accident.
ELIGIBILITY:
Each Active, Full-time employee working 17.5 or more hours per week, and Part-time employee working 17.5 or more hours per week, except any person working on a temporary or seasonal basis.

Effective 10/1/2021, you can choose the level of coverage that fits your needs! Get 70% or 50% reimbursement on vet bills and more.
You may purchase voluntary pet insurance coverage on dogs, cats, and birds. Discounts are given for insuring more than one pet per owner. Premiums can be paid through payroll deductions.
